To do this, you need to fill out the form here to register:. It should include your name, age, address and contact phone number. It is noteworthy that in the form you can name which vaccine you prefer. However, it is unclear whether the city guarantees that you will be vaccinated with the exact drug of your choice.
After registering, you will be contacted by phone from a vaccination program representative to schedule an appointment for your vaccination.

“This is really important for people who are ready to be vaccinated, but for whom it was a problem to get to the vaccination site,” said Mayor Bill de Blasio. "The life-saving vaccine is now available right in your home."
The mayor said that 15 residents of the city have already been vaccinated at home.
